This sidewalk needed to be ripped out and replaced to be ADA wheelchair compliant. We poured it 6" thick and through in a 5/8" rebar bi-directional grid, set in epoxy, to insure it never gave any problems.
This is how it turned out. I just dumped out the water in the bucket holding my caution tape:
